Scaled exchanger on hard Tri-Valley water. The hard inland water scales the heat exchanger and you notice slow hot water and temperature drift. I descale annually with a circulation pump and check flow and mixing after, adding isolation valves where the builder left them off.
Flame rod and igniter fouling after several seasons. After a few years the flame sensing rod or igniter fouls and the Navien fails to light cleanly or drops out under load. I clean or replace the rod and igniter and confirm a steady flame signal, rather than replacing the whole gas valve assembly.
