Undersized gas branch on converted rental units. Most of the duplexes and fourplexes I see in East Palo Alto had a tank heater originally, and the gas branch feeding it stayed the same size after a Navien went in. Under full fire, especially with a couple of units in the building running hot water at once, the unit cannot pull enough gas and locks out on ignition or flame loss. I measure pressure under real load and upsize the branch where it is short.
Clogged inlet filter from an unmaintained supply line. A lot of these units have gone years without anyone touching the inlet water filter, since nobody in a rental thinks to. Sediment builds up, flow drops, and the unit can fault on low flow even when the water pressure at the street is fine. I pull and clean the inlet filter and show the tenant or landlord where it is so it gets checked again.
Simultaneous-draw ignition faults in multi-unit buildings. In a fourplex where two or three units can pull hot water at the same time off one Navien, the demand spike the burner has to chase shows a marginal gas supply or a partially fouled burner faster than a single fixture ever would. I test the unit under a real multi-fixture draw, not just a single tap, since that is closer to how the building actually uses it.
